Forms for Mambo

This is an oldie but a goodie.  This software displays an HTML form on the page you select, and delivers the content via email.  It optionally displays a "thank you" message or redirects you to a content page of your choosing.  This (AFAIK) works for Mambo only.

Geocoded Postal Code database

This CSV has all US Zip codes with their corresponding latitude and longitude, city and state. It also has all country latitude and longitude including Canadian provinces.
